An outbreak of fungal meningitis has been linked to steroid shots for back pain made by a pharmacy in Farmingham, Massachusetts.  The name of the pharmacy is New England Compounding Center.   The medication was recalled in late September and has been attributed to 419 patients diagnosed with deadly fungal meningitis and 30 deaths.

As of November 5, 2012, the Centers for Disease Control and Prevention CDC is reporting:

Illnesses: 419, including 10 joint infections from steroid.

Deaths: 30 from steroid injection.

There are 19 states affected by the recall and fungal meningitis including; Florida, Georgia, Idaho, Illinois, Indiana, Maryland, Michigan, Minnesota, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Rhode Island, South Carolina, Tennessee, Texas and Virginia.
